Single Address Ticket can be used by a homeowner, contractor, or anyone who needs to request utility lines to be marked at a single address. The form is designed for simple requests. No training needed.
Single Address Tickets are valid for 15 business days and will need to be updated on the 12th business day. The ticket only needs to be updated if the excavation will continue past the 15 business days. The Good Thru and Update By dates are listed at the bottom of your email confirmation.
Design/Survey and Emergency tickets may not be processed through this program.
Homeowner: An individual that resides in the home in which the excavation will take place.
Contractor: A company and/or person hired to perform the excavation or demolition work.
Municipality: A city, town, or village that provides water, sewer, electric, gas, telecommunications, or other utilities.
Government: Government operated offices such as a military base or Department of Transportation.
Other: To be used when all other excavator types do not apply.
Utility: An owner/operator that provides water, sewer, electric, gas, telecommunications, or other utilities.
