LOCATE REQUEST SPECIFICS
County: Specify the county name in which the excavation will occur.
Place: Specify the place name in which the excavation will occur.
Subdiv: Specify the subdivision name or business name where the excavation will take place.
Lot #: Specify the lot number(s) if available, use a comma to separate each lot number.
Posted: Click this button if the given lot numbers are posted on the jobsite.
Street: Physical address and/or street name where the excavation will occur.
Cross St 1: Enter the name of the nearest intersecting street to the excavation site, preferably within a ¼ mile.
Cross St 2: Enter the name of an additional intersecting street, if known. This is very helpful if requesting the street to be located from Cross street 1 to Cross street 2.
Accurate information should be provided on locate requests at all times to ensure the correct members are notified. Cross streets within ¼ mile of the job site should be provided whenever possible.
- Individual locate request must be submitted for:
- Each street requesting to be located
- Each intersection requesting to be located, and may include dig site area extending outside of the four corners of the intersection, not to exceed ¼ mile (1,320ft) in total distance.
- Each building needing located (business or apartment)
- Each lot number if the lots are not assigned a physical address, are not adjoining.
- The lesser of five adjoining parcels identified by addresses, not to exceed ¼ mile (1,320ft) in linear length OR an area not to exceed ¼ mile (1,320ft) in linear length.
- If the excavator is not certain the parcels are adjoining, not aware of addresses for the parcels, or not aware if the distance exceeds ¼ mile (1,320ft) in linear length, a separate ticket will need to be issued for each parcel.
- If an address contains a letter (125-A), enter the numbered address in the address field, and enter the number and letter in the Locate Info field of the ticket.
- Distance requested to be located per ticket must be limited to an area not to exceed ¼ mile (1,320ft) in linear length.
- Include and abbreviate all road endings and directionals. Road names cannot be abbreviated.
Road name data must be entered in a specific format in the designated STREET and CROSS STREET fields of the ticket for them to be recognized by the application. See the listing below for a detailed description of how the road should be entered in the Street and Cross Street fields. *Entered road name data in any other format than listed below can cause incorrect members to be notified.
Highways
Highways are to be entered as follows:
- HWY421
- OLD HWY421
- HWY421 BUS
- HWY421 ALT
- If the road name is a multiple highway number, (e.g. HWY15/501), enter one of the highway numbers in the street field and the other highway number in the location field. Enter the nearest intersecting street in the cross street field.
- Directionals (N, S, E, W) cannot be used with highways
Interstates
Interstates are to be entered as follows:
- I95
- I40
- I77
- I85
- Directionals (N, S, E, W) cannot be used with interstates
State Roads
State Roads are to be entered as follows:
- SR1234
- SR4321
- SR9876
- SR4567
- Directionals (N, S, E, W) cannot be used with state roads
Road Names/Endings/Directionals
- Road names must be spelled out and not abbreviated:

- Mount Hope Church Rd (correct) v/s MT Hope CH Rd (incorrect)
- Saint Thomas Ave (correct) v/s ST Thomas Ave (incorrect)
- If applicable, road directionals should be provided and abbreviated (N S E W)
- W Meadowview Rd SE (correct) v/s West Meadowview Rd SE (incorrect)
- N 3rd Street Drive Pl NW (correct) v/s N 3rd ST DR PL NW (incorrect)
- Road names including initials and/ or acronyms ALWAYS include spacing:
- A C C Blvd (correct) v/s ACC Blvd (incorrect)
- A L Smith Dr (correct) v/s AL Smith Dr (incorrect)
Unknown
When a street is unnamed or unknown, “UNKNOWN” should be posted in the STREET field.
Directions to the “UNKNOWN” street should be provided within the Location field of the ticket entry screen. Note : ”Unknown” should never be posted in the Cross Street field.
