REPORT OF ALLEGED VIOLATION OF UNDERGROUND
UTILITY SAFETY AND DAMAGE PREVENTION ACT

The report form provided here was developed by the North Carolina Underground Damage Prevention Review Board.

If you need a resource for identifying the contact information for the registered agent of any company licensed in North Carolina, you may search at the Secretary of State.

The North Carolina Underground Damage Prevention Review Board and the North Carolina Utilities Commission are responsible for enforcement of the Underground Utility Safety and Damage Prevention Act. 

Within 10 days of receipt of a report of alleged violation, NC Underground Damage Prevention Review Board will contact any person who is the subject of a report to inform them of the alleged violation. Please provide complete contact information for the person who is the subject of the report so that NC Underground Damage Prevention Review Board can notify them about the alleged violation.  If you do not provide complete contact information for the person(s) who are the subject of your report, NC Underground Damage Prevention Review Board may write to you to request the address in order for NC Underground Damage Prevention Review Board to be able to contact such person(s) about your report of alleged violation.

North Carolina 811, Inc. does not monitor reports of alleged violations or response to reports of alleged violations, nor will North Carolina 811, Inc. be able to provide you information about the status of your report of an alleged violation of the Underground Utility Safety and Damage Prevention Act. 

If the NCUDPRB sends the case to the NC Utilities Commission, track that docket by going to https://starw1.ncuc.net/NCUC/page/Dockets/portal.aspx. Select “Limit search to the selected docket types”, highlight UDP – Underground Damage Prevention, enter a date range and select search.
